Include a statement that you have a good faith belief that the use of the material is not authorized by the copyright owner, its agent, or the law.
Example: "I have a good faith belief that the use of the copyrighted material described above is not authorized by the copyright owner, its agent, or the law."
Include a statement that the information in your notice is accurate, and under penalty of perjury, that you are the copyright owner or authorized to act on behalf of the copyright owner.
Example: "I declare, under penalty of perjury, that the information in this notification is accurate and that I am the copyright owner or authorized to act on behalf of the owner."
Sign the notice with your physical signature (if sending by mail) or electronic signature (if sending by email).
Example: "/s/ John Doe" or simply "John Doe" at the end of your email.
